As part of counseling and integrative psychiatry, we can help with many documentation requests such as FMLA, short-term disability, and school or workplace accommodations when clinically appropriate. Paperwork is completed by your treating clinician after an assessment and may not be available for every request. Please note that fees apply for completing documentation requests. These services are billed based on your clinician’s standard hourly rate.

Counseling often comes with real-life paperwork. At Kellen Mental Health, your therapist or psychiatric provider can complete many common forms when they relate directly to your care. This can include FMLA and short-term disability paperwork, school or workplace accommodation forms, and letters that verify attendance or ongoing treatment.
To protect your privacy and keep documentation accurate, we complete forms only after we have enough clinical information. In most cases, that means talking through your request in a session, clarifying what the form is asking, and making sure we have your written permission to share information with an employer, school, or other third party. We use a secure Client Portal to exchange forms and return completed documents in a confidential way.
Whether you are in individual therapy, couples and marriage counseling, family therapy, or online therapy and virtual counseling, we will help you request only what is needed and keep the focus on your well-being. If your documentation is related to anxiety and stress management, depression counseling, trauma and PTSD therapy, ADHD support, postpartum depression and maternal mental health, or LGBTQ+ affirming counseling, we will work with you to describe your needs clearly and respectfully.
