How do I know if my insurance will reimburse me—and can you provide documentation (like receipts or a superbill) to submit?

Upon request, a monthly superbill can be provided for you to submit to your insurance company for possible out-of-network reimbursement.

Counseling and integrative psychiatry services provided at KMH may be covered in full or in part by your health insurance or employee benefit plan; however, coverage varies by plan, and KMH is not responsible for verifying insurance benefits or determining reimbursement eligibility. Upon request, we are happy to provide a monthly superbill for you to submit to your insurance company for potential out-of-network reimbursement.

Please note that insurance companies require a mental health diagnosis in order to process reimbursement. This diagnosis must be included on the superbill and will be shared with your insurance provider when you submit your claim. Coaching services, wellness services, workshops, and any testing recommended through third-party companies (including neuroendocrine or pharmacogenetic testing) are not eligible for insurance reimbursement.

We encourage you to contact your insurance provider prior to scheduling your initial appointment if reimbursement is important to you. Please be aware that time spent on insurance-related requests exceeding 15 minutes (including calls, forms, or additional documentation) is not included in your session fee and will be billed at your provider’s standard hourly rate.

If you are 18 years or older and would like your superbill sent directly to a parent or another party, a signed Release of Information (ROI) form must be completed before we can share that documentation.
